Membership Terms & Conditions
Payment Authorization:
I authorize Slope Wellness to charge my credit card indicated above for the amount indicated above; if a monthly payment plan was selected above, my credit card will be automatically charged each month for the amount indicated above for the duration of the membership. This authorization is to remain in effect until Slope Wellness has collected all fees associated with the terms and conditions of this Agreement. Members are required to notify Slope Wellness of any changes to payment information on file.
Membership Term & Renewal:
The Member named above has signed up for a 6 month or 1 year membership term as indicated above. Member is liable for all fees associated with the full term of this Agreement. After the initial membership term has expired, all monthly payment membership plans shall automatically renew on a month to month basis at the prevailing monthly rate (subject to change) until 30 days written notice of cancellation is provided to Slope Wellness.
Freezing Membership:
Members may freeze their membership under certain conditions subject to approval by Slope Wellness management. Examples of extenuating circumstances that freezing would be allowable include extended illness, military leave, or temporary relocation. A membership may be frozen for a minimum of 30 days and a maximum of 90 days. During the freeze period, Members may not accrue or use any membership benefits. The Member may end freeze at any time at least 30 days after the start of the freeze or freeze will end automatically on pre-determined freeze end date. If on a monthly payment membership, once freeze has ended your credit card will be charged immediately for the next monthly payment and this will be your new recurring monthly payment date. Your membership expiration will be extended for the amount of time your membership was frozen.
Membership Cancellation Policy:
Memberships may be terminated for the following reasons: (1) If you move your permanent residence outside of the 5 boroughs of New York city (proof of move required), (2) if you become permanently disabled or have any other medical reason why you cannot receive massages (a letter from your doctor is required). Thirty days written notice is required for all membership terminations.
Membership Transfer:
Upon approval from Slope Wellness, Member may transfer their membership to any eligible and approved person. Thirty days written notice is required for any membership transfer request. Slope Wellness may freely transfer the membership benefits and obligations under this Agreement.
Appointment Cancellation & Rescheduling Policy:
Members agree to Slope Wellness’s appointment cancellation and rescheduling policy. Slope Wellness requires 24 hours notice to cancel or reschedule any appointments. Appointments cancelled or rescheduled after 24 hours are subject to a cancellation fee equal to 50% of the service fee.
Membership Sharing:
Members may share their included monthly massages with eligible friends and family for the Frequent Membership plans only. “Eligible friends and family” are individuals that have been approved by both the Member and Slope Wellness.
Membership Benefits:
Members are entitled to the “included benefits” stated above for the membership plan they selected. Member’s benefits are only valid for use during the period that their membership is active and in good standing. Members are allowed to rollover unused included monthly massages into subsequent months for as long as their membership is active and in good standing. Once membership has expired or has been cancelled any unused included monthly massages are no longer valid.
